    MADEwkshop RiNo is located on the corner of 36th and Walnut Street in Downtown Denver’s RiNo Art District. Our address is 3563 Walnut Street, Denver, CO 80205.

  • Age Requirements: Classes, Workshops, and Private Lessons
    • Classes, workshops, and private group experiences are available for Adults ages 18+.
    • Private Lessons (singles, duos, trios, and quads) are available for adults ages 18+,
    • Private Lessons (duos, trios, and quads) are available for students 13+ with a participating parent/guardian (1 adult for every 2 children required.)
    • Single classes and workshops are 2 or 3 hours in duration, depending on the project.
    • Series project workshops consist of two to four sessions, 3 hours per session.
    • Team Building and Private Group workshops start at 3.5 hours per session. Advance booking required.

    Private Lessons and Specialty Privates must be booked in advance, and a 3-hour minimum reservation is required.

  • Tools and Equipment
    • We offer a full suite of metal shop, welding, woodworking and specialty equipment related to programming.
    • We do not offer 3D printers, CNC machines, or other robotic equipment.
  • Metal Shop + Welding Basics. We’re just scratching the surface.
    • MIG Welding is by far the most popular and accessible form of welding – there’s a little bit of “point and shoot” to the process, and it’s often compared to using a glue gun. Trust us – it’s not that simple, but it sure is fun.
    • TIG Welding is considered the most artistic and precise of all welding disciplines. It offers (and requires) high levels of control and can be used on almost any type of metal. There’s a whole “stack of dimes” thing too. Ask any of our instructors – they’ll chew your ear off waxing poetic on the topic. Don’t say we didn’t warn you.
    • Stick Welding is most commonly used when strength is needed – think construction, furniture building, and sculpture. It definitely creates sparks.
    • Forging is a pretty cool process used to form and shape metals using varying levels of heat while hammering, pressing or rolling metal. There’s a whole show about it called “Forged in Fire” on The History Channel. Check it out.
  • Woodworking Basics and Fun Facts. We could go on for days.
    • Cuts are a big deal, and below you’ll find info on 3 different cut types typically made with a table saw or a miter saw. But then again, there are SO MANY types of saws…
      • Straight cuts: crosscut, rip cut, and resawing
      • Angled cuts: miter cuts, bevel cuts, and tapers
      • Non-through cuts: dado, groove, and rabbet

    • The epoxy trend is going nowhere. Also known as resin, epoxy is basically liquid plastic poured into reservoirs in wood, and the resulting effect can change the entire look of your project. If you like that kind of stuff. Which we currently do.
    • Woodturning is REALLY cool. It’s the craft of using a wood lathe with handheld tools to cut a shape and form an object. Like pens, bowls, and all sorts of crazy good stuff.
    • There are about a gazillion tools in a typical woodworking shop, but the table saw is probably one of the most important. We’re huge fans of SawStop’s table saws, because quite frankly, their hot dog demo says it all. Google it. Unless you’re squeamish.
  • causeMADE Basics
    • Each month (or quarter), we partner with a different local nonprofit.
    • From a list we provide, our partner nonprofit selects an item that serves their functional needs.
    • The selected item becomes our causeMADE project of the month.
    • Anyone (18+) can sign up to make for the cause – choose from one or all four of the scheduled sessions.
    • Finished projects are signed by each participant, donated and delivered to our nonprofit partner.
    • Need a little CSR for your team or a private group? Just tell us how many people and the cause you care about – we take care of the rest. All your group has to do is show up with a smile and be ready to make. It’s THAT easy.
  • Does MADEwkshop carry liability insurance?

    Yes! We have liability insurance, and we also require members and all participants to sign a liability release waiver, both online and on-site. We also insist that everyone who enters the shop be respectful of our instructors, staff, other participants, and the tools and equipment at all times. We will always do our best to promote a safety-conscious culture at MADEwkshop.

  • Materials, Equipment, and Instructions

    All materials, equipment, and instruction are included in the cost of scheduled classes and workshops. For team building, private groups, events, private lessons and specialty privates, upgraded materials are available upon request for an additional charge.

  • Do you rent tools/workspace for use?

    Not at this time. We offer guided classes, workshops, team building and group events, and private lessons – all led by our experienced artists and instructor guides.

  • Are kids allowed at MADEwkshop?
    • Currently, we offer private lessons for kids and young adults ages 13-17 provided they are accompanied by and under the direct supervision of a parent or guardian. Signed waivers are required for all participating students.
    • In the future, we plan to offer after-school and week-long immersive Youth Programs for ages 13-17. For more information on these programs, please drop us a note using the form on the Contact Us page.
  • Accessibility

    MADEwkshop is wheelchair accessible. Each workshop space features two wheelchair-friendly / chair-friendly workstations.

  • How do I cancel or get a refund?

  • What to wear:
    • Currently, we require all students wear a mask that covers their nose and mouth while at MADEwkshop
    • Wear all-natural fibers clothes (cotton, wool, leather).
    • Long sleeve shirts are required.  Pants/jeans without holes / tears. Once in the shop, you will need to cover exposed areas with duct tape, so plan accordingly.
    • No open toe shoes allowed. Please wear all leather or canvas shoes that cover your foot entirely (no clogs, sandals, or flats). Please leave shoes made of mesh and synthetic fabrics at home (ex: running shoes).
    • Long hair must be tied back. Bring a hair tie, just in case.
    • Please leave your jewelry / watches at home. If you forget  – we all do sometimes! – we simply ask that you store them in your pocket or someplace safe.
    • All the other safety equipment (safety glasses, goggles, helmets, gloves, etc.) are provided.

Policies

  • Payments
    • Full payment is required at booking to reserve your class spot, private lessons, or private group sessions. All sales are final. Terms + conditions apply.
    • Gift cards are not refundable or redeemable for cash.
    • Sign up with 5 or more people to secure any scheduled time slot and get 10% off. Advance payment in full is required. Terms + conditions apply.
  • Class Policies
    • Instructors are subject to change without notice.
    • Please arrive at least 15 minutes prior to class start. We cannot accept participants after the start time of the class.
    • Participants are not permitted to enter the workshop spaces more than 5 minutes after the scheduled start time. No exceptions. If you are later than 5 minutes past class start time, we cannot offer you a credit or refund for the class.
    • Glass bottles are not permitted in the workshop areas.
    • Cellphones are permitted in class – but only to take photos and videos using the equipment provided by MADEwkshop AFTER being set up per the lead instructor’s instructions. IF YOU CHOOSE TO BRING ELECTRONIC DEVICES INTO THE WORKSHOP AREAS, PARTICIPANTS AGREE TO HOLD MADE WORKSHOP HARMLESS FROM ANY DAMAGE THAT MAY OCCUR, AND AGREE TO ASSUME FULL FOR YOUR RESPONSIBILITY FOR YOUR PERSON AND PERSONAL PROPERTY.  All electronics must be set to Airplane mode while in class. Our front desk number is available in case of emergency, and a member of our team will retrieve you to take the call outside the shop area or relay the message. MADEwkshop is NOT responsible for any damage to any personal cell phones / tablets / electronic equipment brought into our facilities.
    • MADEwkshop reserves the right to cancel a class due to low attendance. Registered participants will be notified within 24-hours prior to class start and be given a credit and alternative date options.
    • All participants agree to follow all safety rules and guidelines, acknowledge that MADEwkshop programs involve the use of power and hand tools. The use of these tools and materials include risks that may lead to injury.
    • All participants agree to immediately receive first aid and/or medical treatment should the need arise, and to submit to a drug/alcohol test during treatment if asked.
    • Safety is our #1 priority, and MADEwkshop is a sober activity. If it is determined by our team that a participant appears to be compromised or behaving in a manner that could cause harm to themselves or others, you will be immediately asked to leave the facility, and no refunds will be given. If you have a medical condition that presents in a manner to this effect, please contact us at least 72-hours prior to your scheduled class to speak with a member of our customer experience team. No exceptions.
  • Memberships, Cancellations and Holds
    • Memberships include two 3-hour classes or three 2-hour classes per month (6 hours total); 10% off all retail, additional classes and private lessons; and advance invites for specialty workshops and member-only events.
    • All Memberships are auto-pay and renew monthly after first use.
    • Memberships require a minimum 3-month commitment.
    • New members receive a one-time $99 rate for the first month and are charged $199/mo auto-pay thereafter.
    • Memberships require a 30-day written cancellation notice via email to hello@madewkshop.com. You will receive confirmation upon receipt along with your final payment date. The 30-day policy is based on the membership billing date.
    • Membership Hold. Memberships are eligible for our hold program. You may place your membership on hold one time per calendar year. All membership suspensions will be capped at a 3-month maximum time allowance. After the 3-month term, your membership will be reactivated. For each additional hold, there will be a fee of $40. MADEwkshop must obtain a request to cancel 30 days prior to the final payment.
    • Military Deployment Hold: Any member that serves in any branch of the armed forces may suspend their membership for the entire length of their military deployment.
    • All Members are eligible for 2 courtesy cancellations per calendar year, per the terms + conditions of our cancellation policy.
    • All Membership sales are final.
  • Packages, Cancellations, and Conditions
    • All Package sales are final.
    • All Packages are good for 6 months from the date of purchase.
    • All Package holders are eligible for 1 courtesy cancellation per calendar year, per the terms + conditions of our cancellation policy.
    • Discounted packages are available for students, teachers, and members of the military (active and retired). Proper and current legal documentation is required to activate discounted rates.
  • Cancellations and Refunds
    • All participants must agree to the cancellation policy in order to attend MADEwkshop. No exceptions.
    • All sales are final.
    • Classes are not eligible for transfer, exchange, or refund.
    • Cancellations made less than 5 days before a scheduled are considered a late-cancel are non-refundable.
    • Cancellations made 5 or more days before class start can be rescheduled at no charge, class selection pending availability.
    • The first class of a multi-session class covers important material. If you cannot make it to this, you’ll need a catch up with a private lesson ($95/hr.) before continuing on with the class series.
    • No-shows are non-refundable and forfeit the right to reschedule.
    • There are no refunds or credit given at any time for classes with out-of-town and/or special guest instructors.
  • Waivers / Liability Release
    • All participants are required to sign all waivers: the first online during class registration, and a second upon arrival at the shop at check-in. No exceptions.
    • For participants under 18: parent/guardian signature required on both online and on-site waivers.
    • All participants are required to have a current waiver and emergency contact on file prior to the start of each class.
    • Upon creating an account or purchasing any MADEwkshop program, you are acknowledging and agreeing to the below Participant Liability Waiver and Statement of Financial Responsibility Release.


    WARNING: READ CAREFULLY. THIS AGREEMENT INCLUDES A RELEASE OF LIABILITY AND WAIVER OF LEGAL RIGHTS THAT DEPRIVES YOU OF THE RIGHT TO SUE MADE WORKSHOP LLC, D/B/A MADEWKSHOP (“MADEwkshop”) AND OTHER PARTIES. BY ACKNOWLEDGING THIS DOCUMENT, YOU INDICATE YOU ACCEPT IT AND HAVE READ IT IN ITS ENTIRETY. SEEK THE ADVICE OF LEGAL COUNSEL IF YOU ARE UNSURE OF ITS EFFECT.

    I understand and acknowledge that there are risks while participating in activities and using the facilities offered by MADEwkshop. By registering for classes/workshops through MADEwkshop and/or by participating in any MADEwkshop class/workshop, or by purchasing any MADE at Home MADEwkshop Kit, I agree to the following Participant Liability Waiver and Statement of Financial Responsibility. I understand, acknowledge, attest, agree and represent as follows:

    SAFETY AND CONDITIONS OF USE
    I agree that I am personally responsible for my safety and actions while at MADEwkshop or by purchasing any MADE at Home MADEwkshop Kit. I agree to use MADEwkshop facilities, tools, equipment, and materials, and MADE at Home   MADEwkshop Kits in a safe way, and to utilize safety gear appropriate for each task in which I engage, including, but not limited to, eye protection at all times when in the work areas. If I have any questions as to the proper or safe use of any tool, equipment, or material, I will not use the same until I have consulted with MADEwkshop personnel. I will comply with all MADEwkshop policies and rules, including but not limited to all MADEwkshop guidelines, signage, and instructions. If at any time I notice machinery, equipment and/or tools are not functioning properly, I agree to stop using said machinery and immediately contact a MADEwkshop staff member. If I see anyone acting in an unsafe manner, I agree to immediately contact a MADEwkshop staff member. I agree that I shall be responsible to pay any applicable fees for use of MADEwkshop facilities, tools, equipment, or materials, and for any and all damage, loss, or clean-up caused by my use of the same. Because MADEwkshop is open for use by other individuals, I recognize that confidentiality is not and cannot be promised. I understand that I am responsible for properly monitoring and labeling anything I bring into MADEwkshop, and that MADEwkshop is not responsible for any lost, damaged, or stolen property.

    ELECTRIC ARC WELDING IN USE IN FACILITY.
    Electric arc welding and cutting processes produce intense electric and magnetic (electromagnetic) fields. Persons with a pacemaker should NOT go near welding or cutting operations until they have consulted their doctor and obtained information from the manufacturer of the device. I attest that I do not have a pacemaker and personally assume all risks.

    ASSUMPTION OF THE RISK
    I recognize and agree that my use of MADEwkshop facilities, tools, equipment, materials, and MADE at Home MADEwkshop Kits is entirely optional and voluntary. I further understand and agree that such use involves various inherent hazards, dangers, and risks, including without limitation, and only by way of example, the risk of trips, slips, and falls; cuts, broken bones, burns, and other wounds to hands, head, feet, eyes and other body parts; serious personal injury and illness, paralysis, permanent disability, and even possibly death; electrical shock; impalement; exposure to dust, fumes,
    smoke, noise, and vibrations; and accidents due to negligence of other users or MADEwkshop personnel or vendors, or due to defective or inadequate facilities, equipment, tools, machinery, materials, or due to inadequate maintenance or repair, training, instructions, supervision, first aid, and medical treatment, or safety gear. I have no known medical or physical condition (including pregnancy), mental or emotional disability including but not limited to cardiac irregularity, episodes of dizziness, asthma or other breathing difficulties, diabetes, neurological disorders, spinal joint tendon or ligament injuries, high/low blood pressure or any other disability that would put me in any physical or medical danger or prevent or limit me from attending and/or participating in any MADEwkshop classes. I ASSUME FULL RESPONSIBILITY FOR ANY RISKS OF LOSS, DAMAGE, OR PERSONAL INJURY, INCLUDING DEATH, THAT I MAY SUSTAIN, OR ANY LOSS OR DAMAGE OF PROPERTY I OWN, AS A RESULT OF BEING ENGAGED IN SUCH ACTIVITY, WHETHER CAUSED BY THE NEGLIGENCE OF MADEWKSHOP OR OTHERWISE.

    RELEASE OF LIABILITY
    With full awareness and appreciation of the risks involved, I, for myself and on behalf of my family, spouse, estate, heirs, executors, administrators, assigns, and personal representatives, hereby forever release, waive, discharge, and covenant not to sue MADEwkshop, its managers, members, officers, agents, servants, independent contractors, affiliates, employees, successors, and assigns (collectively the “Released Parties”) from any and all liability, claims, demands, actions, and causes of action whatsoever, directly or indirectly arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or any of the property belonging to me, whether caused by the negligence of the Released Parties, any third-party using MADEwkshop, or otherwise, while participating in any activity while in, on, or around MADEwkshop and/or while using MADEwkshop facilities, tools, equipment, or materials.

    INDEMNIFICATION
    I agree to indemnify, defend, and hold harmless the Released Parties from and against any and all costs, expenses, damages, claims, lawsuits, judgments, losses, and/or liabilities (including attorney fees) arising either directly or indirectly from or related to any and all claims made by or against any of the Released Parties due to bodily injury death, property damage, loss of use, monetary loss, or any other injury from or related to my use of MADEwkshop facilities, tools, equipment, or materials, whether caused by the negligence of the Released Parties or otherwise. In the event of an illness, injury or medical emergency arising during the program or while I am at MADEwkshop, I hereby authorize and give my consent to MADEwkshop personnel to secure from any accredited hospital, clinic and/or physician any treatment deemed necessary for my immediate care. MADEwkshop may, but shall not be obligated or required to, administer to emergency aid, CPR, and use an AED (defibrillator), secure emergency medical care or transportation (i.e., EMS). I agree that I will be fully responsible for payment of any and all medical services and treatment rendered to me including, but not limited to, medical transport, medications, treatment and hospitalization and agree to indemnify and hold harmless the Released Parties from any liability associated with the treatment or related expenses.

    ACKNOWLEDGMENT
    I acknowledge and represent that I have read the foregoing waiver, understand it voluntarily as my own free act and deed, including without limitation the Release of Liability and Indemnification requirements contained above; I am sufficiently informed about the risks involved in using MADEwkshop; no oral representations, statements, or inducements, apart from the foregoing written agreement, have been made. I agree that this waiver shall be governed by and construed in accordance with Colorado law, and that if any of the provisions hereof are found to be unenforceable, the remainder shall be enforced as fully as possible and the unenforceable provision(s) shall be deemed modified to the limited extent required to permit enforcement of the wavier as a whole. Any dispute related in any way to my participation in a MADEwkshop program or this waiver shall be submitted to confidential arbitration in Denver, Colorado. Any such arbitration shall be conducted under the rules then prevailing of the American Arbitration Association. The arbitrator’s award shall be binding and may be entered as a judgment in any court of competent jurisdiction.

    RELEASE FOR PERSONAL LIKENESS:
    By entering the MADEwkshop premises and participating in the program, I understand that MADEwkshop and its affiliates, agents, representatives, and assigns may take photographs and/or video during the class of participants and/or their work. I grant to MADEwkshop and its successors, franchisees, assigns and licensees my irrevocable consent to include my photographic image and voice in various marketing materials, including but not limited to social media, websites, flyers, posters, and other promotional materials. All photographs, film or video footage, or other audio or video recordings of MADEwkshop and its participants, and any works derived therefrom, shall be the sole property of MADEwkshop or its assigns, as applicable, and I waive any and all rights of publicity, privacy or other rights therein. If you do not wish to be photographed or recorded, you should not participate in MADEwkshop. I WAIVE THE RIGHT TO INSPECT OR APPROVE VERSIONS OF IMAGES OR VIDEOS USED FOR PUBLICATION OR THE WRITTEN COPY THAT MAY BE USED IN CONNECTION WITH THE IMAGES/VIDEOS. I RELEASE MADEWKSHOP FROM ANY CLAIMS THAT MAY ARISE REGARDING THE USE OF MY STATEMENTS, VIDEOS, OR IMAGES INCLUDING ANY CLAIMS OF DEFAMATION, INVASION OF PRIVACY, OR INFRINGEMENT OF MORAL RIGHTS, RIGHTS OF PUBLICITY, OR COPYRIGHT.

    I REPRESENT I HAD A SUFFICIENT OPPORTUNITY TO READ THIS AGREEMENT, I HAVE READ AND UNDERSTAND THIS AGREEMENT, AND I AGREE TO BE BOUND AS SET FORTH HEREIN. I am at least eighteen (18) years of age and fully competent; and I agree to the terms of this document for full, adequate, and complete consideration fully intending to be bound by the same. This agreement is legally binding.

  • COVID-19 Guest Health and Safety Protocols and Informed Consent

    While MADEwkshop was built for being social with social distancing, we are following additional protocols to ensure the safety of our team, our incredible clients, and our community.

    Outlined below are the guidelines our guests must follow until further notice. Please note that the following is subject to change at any time, and we will update our social media channels with the latest information as it continues to evolve.

    If you are experiencing any signs of illness, have been exposed to COVID-19, or are experiencing a fever, cough, or trouble breathing, please stay home and reschedule your appointment for at least 14 days later to ensure you are symptom-free. Our strict cancellation policy for rescheduling and no-shows still applies, and you will be charged in full. If you are uncomfortable with any of the guidelines outlined below, please notify us immediately at hello@madewkshop.com and we will reschedule your appointment for another time.

    When booking an appointment at MADEwkshop, you are agreeing to all of our policies and full payment for services received or not received. In an effort to make sure our staff receives a full day’s wage, we have a strict cancellation policy, outlined on our website. No shows and short notice cancellations will incur a full charge to guarantee our instructors get paid for their time. We thank you in advance for understanding.

    FAILURE TO FOLLOW MADEwkshop HEALTH AND SAFETY PROTOCOLS WILL RESULT IN THE REFUSAL OF SERVICE, WITH NO REFUNDS GIVEN.

    • We strongly advise immunocompromised, at risk and vulnerable humans including pregnant and nursing mothers to really consider your risks before scheduling your appointment, and highly suggest you do not schedule a non-essential service or activity to reduce your exposure to COVID-19 and any other major health related illnesses and injuries during this time.
    • If you cannot wear a mask for longer than 15 minutes due to any health related reasons, we urge you to not schedule an appointment at this time. Your mask must be kept on during your entire time at MADEwkshop. If it is taken off, the instructor will stop the class/lesson/session/workshop immediately and ask you to leave the premises with no refund.
    • While we are taking extra precautions to maintain a clean and safe environment, by booking an appointment you acknowledge that you’ve read our guidelines and are comfortable with the risks associated booking a non-essential service / activity during this time of transition, and you will not hold MADEwkshop and ANY EMPLOYEES liable for any injury, illness or health conditions that arise from participation in non-essential personal services / activities.
    • If you arrive at your appointment showing any visible respiratory symptoms of illness, even while wearing a face mask – for example, cough, fever, shortness of breath – we have the right to refuse service with NO REFUND.
    • It is mandatory for all guests to wear a face mask that covers your nose and mouth while inside at all times. Please bring your face masks, or we have the right to refuse service with NO REFUND. *Masks will be available for purchase.
    • As always, work gloves will be provided by MADEwkshop for you to wear and take home to keep.
    • As an additional health and safety precaution, ALL Guests and Employees will have mandatory symptom screenings and temperature checks before entering the workshop spaces.
    • It is also mandatory for all our instructors and front desk hospitality team to wear a face mask at all times while inside performing services.
    • We are strictly following all local and state health department procedures, and COVID-19 health and safety regulations and guidelines from the CDC and OSHA.
    • Our workshop areas, helmets, protective eyewear and tools are properly cleaned, sanitized, and disinfected before the start, in-between every session, and at the closing of each day.
    • We will continue to operate “by appointment only” and do not accept any walk- ins.
    • Upon arrival, all guests must use hand sanitizer provided by MADEwkshop in the lobby, both before and after signing our onsite waiver, and prior to entering our workshop spaces. An onsite waiver signature is required for all guests.
    • All participants must also use hand sanitizer anytime they enter or exit the building, and wash your hands for 20 seconds after using the restroom.
    • Please wear CLOSED TOED SHOES AND CLOTHING MADE FROM ALL NATURAL FIBERS. No synthetic fabrics will be permitted in the workshop spaces.
    • Community welding jackets will be provided and are washed daily. If you prefer not to wear a community jacket, please bring a mid-weight flame resistant / fire-retardant long sleeved shirt to wear instead. LONG SLEEVE SHIRTS ARE REQUIRED FOR PARTICIPATION.
    • Please leave ALL personal items in your car other than keys, cell phone, water bottle, any emergency items, inhalers or epi-pens. You will have the option to store your personal items in plastic bins in the lobby that are sanitized before and after each class. You are not permitted to place your personal belongings on any surface in the workshops to minimize spread of contamination.
    • Classes and workshops will have a max capacity of 10 people until further notice.
    • All workstations are at least 6 feet apart.
    • The reception and lounge areas are closed. We ask that you wait outside or in your car until our staff opens the door at your scheduled class/session time. Please note our front entry doors will be locked to limit access of people entering our space.
    • Each workstation, helmet, protective eyewear and tool will be thoroughly sanitized after each session. We have allotted 60 minutes cleaning time in between sessions to ensure everything is above and beyond sanitation and disinfection requirements.
    • Only participants with appointments are allowed in the shop. No friends, pets or extra people are allowed in at this time.
    • After your appointment, participants must check out and leave immediately so we can properly clean, disinfect and sanitize for our next guests. The waiting area/lounge is closed at this time. We ask all participants schedule your next session online.
    • We will continue to require a credit card to schedule appointments or purchase merchandise online.

     

    INFORMED CONSENT

    I understand that the novel Coronavirus (COVID-19) has been declared a global pandemic by the World Health Organization (WHO). I further understand that COVID-19 is extremely contagious and may be contracted from various sources. I understand COVID-19 has a long incubation period during which carriers of the virus may not show symptoms and still be contagious.

    I understand that I am the decision maker for my health and wellbeing. Part of this company’s role is to provide me with information to assist me in making informed choices. This process is often referred to as “informed consent” and involves my understanding and agreement regarding recommended care, and the benefits and risks associated with the provision of health care during a pandemic. Given the current limitations of COVID-19 virus testing, I understand determining who is infected with COVID-19 is exceptionally difficult. To proceed with participating in any active and / or passive activities at MADEwkshop, I agree and confirm that I understand the following:

    • I understand my participation may create circumstances, such as the discharge of respiratory droplets or person-to-person contact, in which COVID-19 can be transmitted.
    • I understand that I am opting for an elective experience, and that I have the option to defer my participation to a later date. However, while I understand the potential risks associated with participating in classes/sessions/lessons/workshops during the COVID-19 pandemic, I agree to proceed with participation at this time.
    • I understand due to the frequency of classes with participants, the attributes of the virus, and the characteristics of procedures, I may have an elevated risk of contracting COVID-19 simply by being in a public space.
    • I confirm I am not experiencing any of the following symptoms of COVID-19 that are listed here: Fever, Dry Cough, Sore Throat, Shortness of Breath, Runny Nose, Loss of Taste or Smell.
    • I understand travel increases my risk of contracting and transmitting the COVID-19 virus. I verify that I have NOT in the past 14 days I have not traveled: 1) Outside of the United States to countries that have been affected by COVID-19; or 2) Domestically within the United States by commercial airline, bus, or train.
    • I am informed that you and your staff have implemented preventative measures intended to reduce the spread of COVID-19. However, given the nature of the virus, I understand there may be an inherent risk of becoming infected with COVID-19 by proceeding with this activity. I hereby acknowledge and assume the risk of becoming infected with COVID-19 through this elective activity and give my express permission to you and your staff to proceed with providing services and instruction.
    • I KNOWINGLY AND WILLINGLY CONSENT TO PARTICIPATION WITH THE FULL UNDERSTANDING AND DISCLOSURE OF THE RISKS ASSOCIATED WITH PARTICIPATING IN PUBLIC ACTIVITIES DURING THE COVID-19 PANDEMIC. I CONFIRM ALL OF MY QUESTIONS WERE ANSWERED TO MY SATISFACTION.
    • I AGREE AND CONFIRM THAT I HAVE READ, OR HAVE HAD READ TO ME, AND UNDERSTAND THE ABOVE COVID-19 RISK INFORMED CONSENT TO PARTICIPATE. I APPRECIATE THAT IT IS NOT POSSIBLE TO CONSIDER EVERY POSSIBLE COMPLICATION RELATED TO PARTICIPATION. I HAVE ALSO HAD AN OPPORTUNITY TO ASK QUESTIONS ABOUT THIS CONTENT, AND BY SIGNING BELOW, I AGREE WITH THE CURRENT OR FUTURE RECOMMENDATION TO PARTICIPATE AS IS DEEMED APPROPRIATE FOR MY CIRCUMSTANCE. I INTEND THIS CONSENT TO COVER ANY AND ALL ACTIVITIES I PARTICIPATE IN (ACTIVELY OR PASSIVELY) AT THIS BUSINESS.
